Error with linux 2.0.x + icmp + pthreads + libc5

This is _not_ a squid patch but it fixes an error a couple of people
have now reported so I'll port it here too (this fix will also appear
in 2.0.37 I hope).

When using libc5 and pthreads, icmp.h gets broken presumably because
of a preprocessor redefinition of errno.

This (untested) patch is against linux 2.0.36 and should make the
problem go away.
